Semen Yuryevich, please tell us how the Heptrong drug appeared?
To begin with, I am a clinician, not a pharmacologist. And our drug began to be developed back in the Soviet Union precisely from a clinical perspective. We were tasked with finding a means to block the liver from toxic hepatitis. At that time, Soviet troops fulfilled their international duty in Afghanistan, and substances were supplied there to combat tropical diseases, which were considered very dangerous. Therefore, employees were given a technical alcohol-water mixture for disinfection, to which dimethyl phthalate was added. This substance does not have its own smell, and therefore, for the lack of high-quality alcoholic drinks, similar alcohol mixtures were consumed by people. The liver was destroyed in three days. The same story was with carbon tetrachloride, which was used to degrease surfaces in the army. By the way, it was written on it, "poison", but since it smelled of alcohol. You understand that. Our army is indomitable, it drinks everything that smells of alcohol. It is impossible to blame our people for this. Although, these are technical alcohol-containing liquids, and in fact ethanol in them is the minimum amount, and the rest are just those substances that destroy the liver. Speaking about all this, one should not forget about the legacy of the Great Patriotic War, about the "people's commissar" 100 grams. All stories have their own background. But the medical problem from understanding the reasons did not become less acute.
And we began to create a drug that would block the processes of liver destruction, at the department of Alexander Popov in Moscow. When we discovered the active substance, which formed the basis of our preparation, the Soviet Union no longer existed. Therefore, we had to move to the department of therapy of Boris Shulutko at the Mechnikov Academy in St. Petersburg and work there.
Has your group managed to cope with the task?
The most amazing thing is that the substance we discovered was isolated from ordinary bee honey, and therefore was not suitable for the post-Soviet space in principle. The fact is that honey contains cyclopentol, which in this form is not found anywhere else in nature. Synthetically, it is rather difficult to isolate cyclopentol, but it can only, it will cost much more than uranium. Therefore, we settled on the fact that we will make it from honey. But here it turned out that honey is not considered a pharmacopoeial substance, that is, a substance from which medicines can be made. By the way, then no state in the world recognized honey as a pharmacopoeial substance. Therefore, it was believed that our drug is "unknown from what." Monstrous stupidity. Everyone knows honey, and mankind has consumed it for millions of years. And, nevertheless, it turned out that honey is "it is not known what." We had to spend more than fifteen years on clinical trials and creating the evidence base for certification and obtaining patents for Heptrong. It was important for us that the drug was recognized as a drug, and not just another dietary supplement. And we had to go through the whole set of clinical trials. After all, when it is known what the medicine is made of, it is checked for a standard set of possible side effects, which is typical for this pharmacological group. But when "it is not known from what", as in the case of Heptrong, then it is necessary to test a new drug for its mutagenicity, for oncogenicity, for its effect on pregnant and lactating women, and so on and so forth. In general, a full set of various and comprehensive studies. In the 1990s, we were unable to continue our research in Russia because of a terrible leapfrog with endless changes in governments, laws, and the Ministry of Health. Although the achievements and results of clinical trials by 1993 were very serious. But then, with our Heptrong, we never entered the Russian market. Today, I also can’t call the situation in the Russian pharmaceutical industry a rosy one. Over the past 10 years, no innovative drug products have been registered in Russia. This is terrible, given that Russia has gigantic research institutes. However, all their designs died before they were born.
In a word, the only country that answered us that it doesn’t matter what we make Heptrong from, if only the result is Uzbekistan. There were people there who were really interested in promoting this drug, because they understood its promise. It should be noted that the people who initially ordered us to develop a new drug, among them was General Leontyev, I don’t remember, unfortunately, his name and patronymic, back in the 1980s they told us, by all means, to bring the work to end, regardless of whether our troops will continue their operations in Afghanistan or not. History proved that the general was brilliantly right. When a price review took place in Russia, if you remember, people began to drink everything - from windshield wipers to other household chemicals, that is, the very poisonous liquids containing alcohol that we die from and that we struggled with, starting to develop Heptrong. Here you have the wheel of fortune. Who knew that everyone would need our designs?
Patients with toxic hepatitis were our first patients. The beneficial effects of therapy exceeded our wildest expectations. When severe patients with cirrhosis began to get to us, it turned out that the drug worked here. We restored what it would seem impossible to restore - the beam structure of the liver.
